- No drug name, no dose and no hour is printed on the report sheet. The hours are yours to write, and what is due belongs to the chart - a drug name copied onto a working aid is a transcription waiting to go wrong
- The hour headings are blank because whether a shift runs 07 to 19, 19 to 07 or eight hours at a time is the unit's business, not ours
- The observation strip on the report sheet is in the same order as the one on the SOAP page, so a set of numbers reads across sheets

What is inside

WHAT MAKES IT DIFFERENT
· A shift sheet — the page the shift actually starts on: the jobs list with a time against each, who to call and their bleep, and the three people you are worried about
· Illness severity is a chip you circle, not a word you write, so it survives being read at arm's length in a corridor — and it carries a letter, so the mono edition loses nothing
· I-PASS and SBAR look different on purpose and each prints one line saying what it is for. Handing a patient over and asking one person for one thing are different jobs
· The observation strip is in the same order on every sheet, so a set of numbers can be read across pages rather than hunted for on each one
· Patient lists come six to a page and four for a heavier patient, each row with a severity chip and three job boxes
· No thresholds, scores or escalation criteria are printed anywhere. Those belong to your unit, and a printed number that disagrees with local policy is worse than a blank box
· No branding inside. You are buying paper, not an advert.
